I also encourage my patients to complete "Hows your health" yearly and to bring it in for review! Click here to complete health checkup survey , then enter IMP407 when prompted. At then end you can have it emailed to me, and you can print it for your records too.

Otherwise please arrive at least 20 minutes prior to your appointment to complete it.  Thanks!!
